The following information in bold italic text is from last week's Business Matters article. Please read below the following article for further clarification:

 

 

2006 ADA Claim Form

The latest ADA claim form is now available in DAISY. The following steps will activate the new claim form.

 

Please note that these steps pertain to users of DAISY 3.0 and higher, and are based on a standard DAISY installation. DAISY 2.4 does not have the new form.

 

1.    From the SERVER, open My Computer.

2.    Navigate to the following location – C:\Program Files\DMC\Reports.

3.    Locate the file "r2211.rpt", right click on it, selecting Delete. Click Yes in the resulting Confirm File Delete window.

4.    Locate the file "r2211_2006.rpt", right click on it, selecting Properties.

5     In the resulting window, remove the text "_2006" from the filename (the field near the top of the window). The resulting filename should be "r2211.rpt" (no quotation marks).

6.    Click OK.

 

Claims printed in your office will now be on the 2006 ADA claim form.

 

Additional Info

The 2006 ADA claim form contains an NPI number field. Previous ADA claim forms do not contain this field.

 

 

Further Clarification

Your NPI is NOT currently stored in the DAISY program, thus your NPI will NOT appear on claim forms printed in your office. If desired, enter the NPI manually after printing the claim. DAISY will soon have the ability to store your NPI, allowing it to appear automatically on printed claims.  Watch your DAISY Inbox for more information.

 

All claims transmitted electronically WILL be processed with the NPI information that you have provided.
