2006 ADA Claim Form

The latest ADA claim form is now available in DAISY. The following steps will activate the new claim form.

Please note that these steps pertain to users of DAISY 3.0 and higher, and are based on a standard DAISY installation. DAISY 2.4 does not have the new form.

1. From the SERVER, open My Computer.

2. Navigate to the following location – C:\Program Files\DMC\Reports.

3. Locate the file "r2211.rpt", right click on it, selecting Delete. Click Yes in the resulting Confirm File Delete window.

4. Locate the file "r2211_2006.rpt", right click on it, selecting Properties.

5 In the resulting window, remove the text "_2006" from the filename (the field near the top of the window). The resulting filename should be "r2211.rpt" (no quotation marks).

6. Click OK.

Claims printed from DAISY will now be on the 2006 ADA claim form.

Additional Info

The 2006 ADA claim form contains a field for the NPI number(s). Previous ADA claim forms do not contain this field.

Starting May 23, all claims will require NPI number(s).
